Flu vaccinations available at the Collinsville Community Health Center,

Flu vaccinations are now available at the Collinsville Community Health Center, 1201 W. Center .Call (918) 596-8650 for clinic dates and times.

There will be a $25 charge per flu shot for all other individuals. Payment can be made in the form of cash or check. 

Flu vaccinations will be available free of charge for the following individuals:

• Children who qualify for the Vaccines for Children (VFC) program
• Medicare Non-HMO recipients,
• Individuals whose household income is at or below 185% of the federal poverty guideline
• Individuals with Health Choice insurance plans
• Medicaid/SoonerCare recipients (does not include SoonerPlan)
